Responding
to a subpoena, supplying records, treatment
summaries or letters to the court will be
charged at the rate of $150.00 per hour.
Insurance
Most
insurances are accepted and with your consent,
I will send in claims to your insurance
company. I am a participating provider for
BC/BS and Tricare and an OON provider for
other insurances.
24-HOUR CANCELLATION POLICY
Because
therapy is scheduled hourly, without adequate
notice of cancellation, an unattended session
is an hour for a therapist which will remain
unfilled while the therapist is there and
ready to work. Therefore, the 24-hour requirement
to cancel an appointment is required for
a therapist to have an opportunity to fill
a cancelled appointment hour.
Unless
a change in plan is due to an emergency,
without 24-hour notification of cancellation,
there will be a charge for the session at
the full session fee.
